GE HealthCare and Biofourmis are collaborating to enable more patients to go home earlier, with the comfort and peace of mind that they are receiving high quality care while they are at home. Moreover, insights from Biofourmis’ FDA-cleared, AI-guided algorithms can help care teams deliver efficient, personalized care at home. The collaboration has three solution offerings.
Hospital at Home offering

Enabling hospital-level care in the comfort of home

The Hospital at Home offering enables you to provide high-quality, cost-effective care for eligible patients at home, while meeting the CMS Acute Hospital Care at Home (AHCAH) guidelines under waiver. The offering provides an all-in-one platform that streamlines program management, allowing module configuration and a detailed view of a patient’s vital signs, ECG, and other physiologic data from FDA-cleared medical devices, as appropriate.

In addition, innovative algorithms generate personalized patient baselines that detect clinical deviation while AI-driven analytics can predict deterioration early, enabling timely interventions.
Timely Discharge offering

Transitioning eligible patients home earlier

The Timely Discharge offering helps eligible patients go home a few days earlier, completing their hospital care plan in the comfort of their home. They’ll be remotely monitored utilizing condition-specific monitoring devices, and will be provided with in-home services, such as phlebotomy, infusion, and durable medical equipment (DME), extending the quality of care to continue at home. Patients eligible for Timely Discharge will be educated on the program, onboarded, and set up on the technology by a nurse liaison.* Additionally, these patients can continue to be monitored by a virtual care clinical team for 30-90 days with the Remote Patient Management offering, helping prevent rehospitalization and improving patient satisfaction.
Remote Patient Management offering

Helping prevent re-hospitalizations

Designed to reinforce health behaviors and patient satisfaction, the Remote Patient Management offering provides high-quality, cost-effective care while patients recover at home. Eligible patients receive required services at home for 30-90 days, receiving personalized kits which may include Bluetooth®/cellular-enabled devices like BP monitors, weight scales, pulse oximeters,𠊊nd more, based on patient care pathway.

GE HealthCare’s FlexAcuity™ monitoring solutions, in combination with GE HealthCare’s virtual care solutions like Mural ICU, Command Center, and Digital CMU, adapt to rapidly changing patient needs in the hospital and are built on a legacy of innovation.
